Everytime you sign up with a new agency you will have to go through the process of submitting all the requested documention that is needed from you in order to create your agency profile. Some agencies may require more or different paperwork from what is listed here. However, for the most part, this is a pretty standard list.

Profile
Your profile is like an electronic book about you, in the computer of the agency. Your profile will have everything from your personal identifiers, to your certifications and licenses, work experience, reference contact information. Your profile is what is submitted to facilities as a candidate for a contract.
Paperwork this is all the documentation that is the 'electronic' you.
Proof of certification
Additional state licenses (all applications will be available for download SOON!)
Healthcare provider BLS (Red Cross© is the most widely recognized and accepted)
Medical Documentation
Titers (blood test that shows proof of immunity to various diseases)
Tetanus vaccine (needed every 7-10 years)
Measles, Mumps Rubella Vaccines (also called MMR)
Chicken pox Vaccine (also called Zoster)
Physical (within last 12 months, unless facility requires 30 days)
Proof of Flu vaccination (every year, prior to flu season)
PPD (TB skin test shows proof of negative for Tuberculosis) Many facilities are requiring the 2-step test
Declaration of Tax Home (in order to be eligible to receive the tax free stipends you must have a declared tax home) You can visit TravelTax.com for an explanation on Tax Home
W4 for state tax withholding
I9 - notarized (proof of identity)
Direct Deposit Form for payroll
Copy of Driver's License- especially, if a company paid rental car is part of pay package
Family Contact (in case of emergency)
Background Check (usually goes back 7-10 years)
Company required online proficiency tests (must be completed prior to contract)
Importance of Organization
Staying organized is very important as a traveler. It is always recommended to have a folder that you take with you on assignments with all the necessary documentation, licenses, BLS and copies of your vaccinations and titers. You never know when you might need them for another agency, especially if you are not registered with an agency that has a contract that you are interested in.
